VISION health plan overview
Vision insurance is designed to help you cover and budget for ongoing vision care expenses like routine eye exams, prescription glasses and contact lenses. You can buy vision plans in addition to your health insurance or even with other supplemental plans, like a dental plan.
VISION PLANS
HORIZON VISION PLANS
All Horizon Vision plans offer:
• An annual eye exam, including dilation
• Coverage for eyeglasses and contact lenses
• A higher frame allowance when purchased through Vision works
• A one-year breakage warranty

The Horizon Vista V plan includes an eye examination for $10. Also coverage for fashion-level frames at no charge, designer level frames with a $15 co-payment, and premier-level frames with $40 copayment. In place of eyeglasses, our plan covers up to $100 for non-collection contact lenses.

The Horizon Panorama V plan includes an eye examination for $10. Also coverage for fashion and designer level frames at no charge, and premier level frames with a $25 co-payment. In place of eyeglasses, our plan covers up to $130 for non-collection contact lenses.
